When attempting to add music to my iTunes library, either by copying from CD or drag and drop MP3s from the finder I get error:
Attempting to copy to the disk failed (-54)
I have tried restarting, and repairing disc permissions.

From an OS9 reference, error -54 is a software lock on a file or a permissions error. (Although there is no formal list of OSX error code some of the old codes still apply to OSX .)
First, try simply restarting your computer.
Try quitting iTunes, then deleting the .xml (not .itl !) version of the library file in the iTunes folder (iTunes will generate a new one).

Try Repair security permissions for iTunes for Mac - Apple Community. Have you checked the health of the drive with Disk Utility?

Non of this seems to be fixing the issue for me. Any other thoughts?
I tried that, it didn’t work. I managed to get it working by making a new iTunes library, but now I have two libraries which I don’t really want.
Does it still work if you revert to the original library?

Nope. Only works with the newly created library.
Reconnect to the original library. Close iTunes. Hold down option+cmd as you launch iTunes to open it in safe mode. Can you now add a new track? Close iTunes and reopen. Any better now?
