my loan company is requesting a credit card statement with my name, address, card number, and balance. where can i get one? the info is on all different screens in the wallet.

If you are referring to the Apple Card, you can download a statement. see the following from How to make Apple Card payments - Apple Support
To view your payment history, follow these steps:
- Open Wallet on your iPhone and tap Apple Card.
- Tap Total Balance.
- Scroll down and select the Statement you want to view.
Next to Payments and Credits you see the amount of Payments and
Credits from the month you selected. If you want to download a PDF of
the statement, tap Download PDF Statement.
