I purchased KOTOR on my MacBook Air 2013. Using the same apple ID it does not appear as purchased on my iPhone 5SE's app store. Do I need to buy the game twice or is there some other problem?

The App stores and apps for the Mac and the iPhone are different. Your Mac apps won't run on your iPhone and vice versa. Most developers charge for each version of their app.
