How will I know if my apple account(iCloud) has been hacked?

What makes you think it's hacked in the first place?

To make it hard

Use a very strong password
Do not reuse the same password for other websites login
Enable 2 Factor Authentication
Do not let someone use your device
Do not modify iOS
