All of my library has been cleared

Have you synced this phone with iTunes or Music app recently? If yes, and the library on the computer was empty, it removed all music on the device and replaced it with the music from computer library (or just wiped everything if the library was empty).

Also, if you sync the device with a new library, it will automatically remove all tracks on the device and replace them with music from the iTunes Library.
