Asking for a neighbor, can she listen to streaming music from her Spotify account while she is out walking without her iphone ? Will it play through the watch speaker so she doesn't have to pair bluetooth headphones?

Playing music through the speaker non-stop would kill the tiny battery very quickly. If you are also using the WiFi radio to stream music, the Apple Watch would be out of power in no time.
The speaker will allow phone calls, walkie-talkie, or similar short conversations. As well as alarms, notices, etc…
note, battery charge does go quicker when on a long phone call, but at least tge speaker is off while you are talking.
She will need headphones.
