I had this going on my 4K in the UK since I bought my iPhone 11 in January. I have recently moved back to Australia, and moved my Apple life to Australia too, and I can't find a way of continuing with the free trial on a different Apple TV, HD. Is this offer territory- or device-specific?

If you changed the Apple ID account region, then the subscriptions from the former region are no longer valid.
The offer is account-specific and region-specific, but not device-specific.
