Create a New User - Does not appear in Users & Groups

I've had a problem pop up twice now on two different 10.14.6 Macs... I created a new Standard user in Users and Groups and the account doesn't show up. But if I try to create the account again, I get a message that it already exists.

If I look in dscl . /Users, the account is there, but doesn't appear on the login screen and won't let me login if I switch to a user/password login. Re-imaging the Mac (with the same base image) fixes the problem.

Any thoughts on this are appreciated, thanks!

This is one of the symptoms of the 2020-005 update.

Did this happen after the 2020-005 SecUpd for Mojave?

Try reinstalling this in Safe Mode, but not using Firefox...

Safe Boot, (holding Shift key down at startup).
