How can I find out which 5 computers are authorized to use my iTunes account?

You can't see the names of the computer, only the number authorized. You can deauthorize all and then authorize any computers you still have. See the information in the "See your authorized computers" section of Authorize or deauthorize your computer for iTunes Store purchases - Apple Support

The Deauthorize all button is shown in the second screenshot in that section.
