I purchased Star Wars: KOTOR on my iPad and I want to play it on my macbook air however, when I search the app on the app store it prompts me to buy the app again. Will I have to purchase the app again just to play it on my macbook?

Your iPad and MacBook Air have two separate operating systems; iPadOS and macOS and most developers develop them separately for each platform and they usually charge for the apps separately for each platform.
