Do you specifically have AppleCare+ with Theft/Loss coverage? If so, then you would file a claim here:

Note that you must have a "Theft and Loss" coverage type. The "standard" AppleCare+ plan does not provide you a new phone without a cost to you. See details about AppleCare+ with Theft and Loss
