How do I request a refund for a subscription that doesn't show up in purchases?

I apparently signed up for a subscription some time ago but didn't catch it until I was catching up on credit card statements. How can I cancel it and request a refund?

It should show in the purchase history of the account you subscribed to it : See your purchase history for the App Store, iTunes Store, and more - Apple Support and http://reportaproblem.apple.com (though purchases are considered final as you know). Do the payments not sure in either place, and the subscription not show in the Subscriptions section of that account ?
