Where has the “Recently Played” Playlist gone in iTunes?

Not sure where it has gone, but you can easily make a new one. File > New > Smart Playlist, and put the rule "Last Played" "in the last" "14" "days." (You can choose a different number than 14 if you wish.)

I don't use it on the phone, so not sure. I would guess that the "Live Updating" for the Smart Playlist only works on the PC, and the playlist on the phone will only get updated when you sync. But can't hurt to give it a try.
