Use the Photo App:

You would open the Photos app. then, wait for there to be lines at the corner of the QR code. Then, tap the button, as if you were to take a photo.

You mean you have a document, which has a QR code on the document itself? If yes, you cannot use the camera to scan it. But you could print the document with the QR code on it, then use your camera to scan it.
