Name appears twice as an iCloud Recovery Contact

I recently setup my wife as a Recovery Contact. I had trouble as first since the instructions weren't clear, but eventually got her added. We are both on the same family plan. The issue I have now is that she is appearing twice as a Recovery Contact. One is grayed out and says "Request Sent". The other is the expected active contact listing. I tried "Remove Contact" on the grayed out one, but it didn't do anything. This was from my iPhone. I my iPad I don't see this extra contact. On OSX I do and I also tried removing it there, but it fails with "Unable to Remove Recovery Contact", and below that "Couldn't communicate with a helper application".

I've tried doing a "Remove Contact" on her active contact (from my iPhone). That works, but the "Request Sent" contact is still there and still can't be removed. On OSX, both entries are still there, but the one that was active is now in the "Pending" state.

From my iPhone, after having deleted my wife as a contact (by the "sent" one remains), I tried to re-add her, but she no longer shows up as a family member I can add (my kids do). I suspect this is because of the pending request that I can't get rid of, so I used it to resend the request. She got the request, but clicking on it says that the "Recovery Contact Invitation Has Expired". I tried this a few times with the same result.

I was eventually able to get my wife added again by using the iPad (and having to use the "Choose Someone Else" route since she was not in the family member list), but still there are extra entries:

  • iPad: just the one correct active entry.
  • iPhone: correct active entry and a grayed out "Request Sent" entry.
  • Mac: correct active entry, "Request Sent" entry (not grayed out), and a "Not Available" entry.
