Up Next is broken

Up Next is hit and miss on my Apple TV Box. It worked flawlessly in the past (except for Netflix, which I understand is always missing) Lately, some shows update and others do not. Surprisingly, many Apple TV+ shows do not update properly or remain on the same episode even though I know for a fact I finished watching the entire episode.

Last week, I did a factory reset and it began working great again but then a few days later, it went back to its old ways of failing to update. Any clues? (yes, I am logged into my apple account and "use play history" is on)
