I am looking to buy a refurbished macbook but need to change the keyboard to have both Arabic and English charcters. Is that possible?

No. Refurbished products are not customizable. They are sold as they are.
You are free to change the input language via System Settings in macOS to Arabic if needed, and the keyboard will act like an Arabic keyboard. The Printed characters on the keys have no actual bearing on the input language. It's just a visual aid when typing. But if you know where the characters are in Arabic on the keyboard normally, you can set the language to Arabic and type in Arabic regardless of what the keyboard characters have printed on them.

True Apple factory refurbs are only available in certain countries and from the Apple online store. Go to the bottom of your country's Apple home page and look for this among the small gray links there:
