You may wish to contact Phonak support about this since it sounds like it could be a Phonak specific issue. Do your hearing aids even connect to your iPhone? Does other audio such as music from a streaming service get supplied to your hearing aids? I have Phonak hearing aids and I know that getting them to work with my iPhone can be finicky at times. I have called Phonak on several occasions over the past year and it was very much a Phonak solution rather than something with Apple's interface.
Have you tried restarting all your equipment?
