Finder file ordering

My Finder in some cases is listing files by file type only which is not what I want. I want the 'Name' column to show but it is missing and doesn't seem to be an option to activate it by right-clicking on the column headers. (See attached). This does not change on any view selected. (multi-column, etc.) How do I restore the 'Name' column in the finder under these circumstances?
