I downloaded a QR code on my IPhone 15 Pro Max last night. After that, I found out that it came from a fake USPS notice (PS Form 3849). Does anyone know what to do next?

Was it a physical form like this? If yes, and you were expecting a USPS delivery, then it's real. No idea though why the carrier would have tucked it into a newspaper. They usually stick or tape them to your front door. Though they sometimes just put them into the mailbox along with the rest of whatever they delivered that day.
No, you don't need a security app. A QR Code can't do anything but lead you to a site, or whatever it does. Some just translate to text.
If you want to check, call the USPS number right on the form. Per the page I linked to, it's 800-ASK-USPS (800-275-8777). I also double checked. It is the legitimate number for the USPS.
Not surprising. The QR code has a use, but is also used for Spam, Ads, and Phishing. The same rules apply, just as if you got an email or message, and I would never enter any personal/financial information.
The only question would be, what information did you provide.
You have a QR code from a notice that simply links to informed delivery? This isn't an issue and possibly a legitimate notice or one perhaps accidentally added by your mail carrier or mail sorting/processing center.
It just goes to my USPS Informed Delivery Account.
They put this notice inside my newspaper. Therefore, that person broke the law. I’m going to notify the Postal Inspector’s Office and the Police Department.
