How can I link an Apple Note inside a Reminder using Shortcuts?

I’m trying to create a Shortcut that automatically generates a Reminder with a direct link to a specific Apple Note.

Does anyone have a reliable method or workflow to generate the Note link and insert it into the Reminder?
