Received return box without return instructions or label

I received a box from Jeffersonville, IN, which I think is what I’m supposed to use to pack up my device and send it back.

But, I’m not sure, as there were

• NO instructions
• NO return label
• NO postage equivalent

Please advise as to what I am supposed to do now.
