To put it simply, there is no real solution to this.
You cannot have more than one Apple Account signed into a device at any one time for the purposes of downloading apps.
When it comes to purchases there are also certain restrictions about frequent changing of signed in accounts --> From: View and remove your devices that are associated with your Apple Account for purchases - Apple Support
"When you try to associate a device that was previously associated, you might notice a message that says, ‘This device is associated with another Apple Account. You cannot associate this device with another Apple Account for [number] days.’ You might need to wait up to 90 days to associate the device with a different Apple Account."
Because Apple may require all information for an account be consistent with a single country you may find it not so easy to create a separate account unless you have separate telephone numbers, mailing addresses, bank cards, etc. for each country.
In the end the people who seem to do this truly do have complete personas in both countries and own two devices, one for each country.
