Open Control Center, and at the top, you'll see the icon for the camera or microphone and which app was using it most recently.
If Control Center displays “Unknown Device” next to the microphone or camera indicator, it may indicate that an accessory connected via Lightning, USB-C, or Bluetooth — such as an external microphone, webcam, or certain headsets — is requesting audio or video input, but iOS is unable to fully identify the device.

No, you haven't been hacked.
What app shows in Control Center?
Also, check Settings>Accessibility>Voice Control and make sure it's set to Off.
There is no Siri indicator light on an iOS device.
Orange dot indicates one of the Apps is open and using the device's Camera

Yes, when you say "Hey, Siri...." you are activating the microphone. However, that isn't the only reason that the microphone might be in use. For example, it will be in use when you are on a phone call.

The only way that an app can listen, or you can talk to the other end of a phone call, or record a voice memo, or that Siri can hear and respond to you, is by using the microphone, and the orange LED shows that microphone usage.

I can't answer any more questions without more information.
Do you have any apps that may be listening even if they are not opened?
If so, that means thats the problem.
If not, it may be lag or your iPhone is hacked (hackers are listening from your microphone)
