Can I setup more than one iCloud email account under one Apple ID?

I have an existing iCloud account. Can I create a secondary iCloud email or do I have to set up an entirely separate Apple ID / Account?

You can create 3 "aliases" for any iCloud email address.

If you're trying to update your account then look at this Change your Apple ID - Apple Support , if you want to use 2 different icloud account on one device, then no.
