Spam phone call? Breached icloud account?

How does Apple let you know if your account has been breached?

I just received a call telling me my icloud account has been breached and to do no more work on line and of course to call their number. Surely a spam call. I hung up. But it did raise questions.

It is indeed a scam call.

Apple does not make such calls.

All such calls are scams from criminals attempting to steal your personal and financial information. The callers will often use number spoofing to pretend they are calling from a legitimate business. The correct course of action is to hang up on them, repeatedly if needed.

Short answer: they don't. Apple never makes unsolicited contact to their users about their account security. The closest thing you would get is an automated email when there is a new sign-in to your Apple ID, or a purchase made on a new device. Even if you get an email like this, you should verify it came from Apple and confirm any links direct to the actual Apple website.
