Deleted App Still appearing with white icon and black lines

I am unable to delete app properly, the app is appearing again and has white icon with black lines. I have tried to restart the phone and download again from app store but i am unable to download(it shows 'open' in app store) means its already downloaded. but when i try to open the app it doesn't even open.

So basically i am unable to delete the app neither download it again, and white icon with black lines with same app name appears but app doesn't work.
