Bluetooth missing from system preferences and "not available" on menu bar

For some time now Bluetooth icon/setup has been missing from system preferences and the menu bar says it is "not available". I have tried Shift+Options and clicking on the menu but no option to fix etc Bluetooth firmware is up to date.

Any suggestions on how I can troubleshoot?
