In order of your questions.

• The Pencil tip should be changed as soon as any wear is noted - or if any part of the enclosed metal transducer is seen.

• The first generation Apple Pencil is supplied with a spare Pencil tip in the box - along with the charging adapter. Additional tips can be purchased in packs of four.

• Simply unscrew the tip (Anticlockwise) from the body of the Pencil; the new tip should be screwed on firmly.

The tip just screws off. Turn it counterclockwise to remove it. If you see any metal/wire at all on the tip, it’s time to replace it. Apple sells them and you can buy them from third party sites. Check out Amazon.
