I don't know when my calendar was deleted.
My calendar was deleted from my iPhone and I don't know how or when. How do I restore it if I don't have the aforementioned information?

First. check it in the App Library (Next to the last page of Home Screen)
If it isn't there, then download from the App Store. --> https://apps.apple.com/us/app/calendar/id1108185179
