Ask the company you bought insurance from. iPhones are not insured, unless you explicitly purchase insurance through your homeowners/Renters policy, your carrier, an independent phone insurer such as Assurion, the store where you bought it, or if you purchased AppleCare+ with Theft & Loss coverage when you got the phone.
