Macintosh HD disk not showing up in Disk Utility

I was trying to reset the Macbook because I had forgot my password but somehow it messed up the disk. Now I am in Disk Utility in recovery mode to try to reinstall a new copy of OS X but there’s no disk for me to select. Disk Utility is showing disk0 and OS X Base Sytem, not Macintosh HD. I’ve seen people say to just erase the OS X Base System but the option to erase is greyed out. Not sure what to do now.

Hi vyfrompflugerville,

Thanks for posting the screenshot.

I'm not too familiar with the Disk Utility found in OS X Yosemite and earlier, but try selecting disk0 and erasing with these options:

Name: Macintosh HD

Format: OS X Extended (Journaled)

Scheme: GUID Partition Table

If the erase options are greyed out, unfortunately your internal hard drive might be dead.
