I have several Roku Ultra devices, and can easily use "private listening" while watching TV to stream the Roku audio to bluetooth hearing aids that are connected to my iPhone. I can't seem to figure out how to do this with the Apple TV 4K. Is this possible?
if the hearing aids rely on a2dp you pair them with the appletv like any other bluetooth headset

This is indeed helpful, thank you. It's disappointing to me that Roku has more advanced iPhone integration than Apple TV. To listen to the audio on my connected device, with Roku--it's one button.
