when it happened to me it was 1 of 4 causes
- dirt / lint in the connector of the ios device, clearing it out with a match or toothpick resolved it
- a damaged cable testing with another cable or testing the cable with another ios device can tell which
- a damaged ac charger, testing with another charger and or a computers usb port would tell which
- a damaged connector in the ios device itself, this would require repair or replacement.
