What if I don't have the device anymore but the device keeps connecting to my iCloud

i had an older iphone with this account and when i switched accounts i was unable to disconnect the original device due to it being broken. every couple of months or so an iphone would appear as being connected to my account and i have to remove it, but theres no notice of anything connected so i dont know if anything has been copied or stolen. i recently had to change my account id and passwords because i was hacked and im not sure where to go from here.

Have you tried using Find My Phone on the phone that appears on your account?

Find My iPhone - Erase your device

Find My Phone - Lock and track your device using Lost Mode.
