When a suspected spam calls reaches my iPhone what message to they receive?

None. Scam calls are not automatically detected or blocked.

If you don't answer then they just hear the ringing but no answer.

If you do answer they get you.

If you block the number, then they still hear the ringing on their side, but you get no call.
