A. Finding Out the Model:

1. Go to: Settings
2. Tap: General
3. Tap: About
4. View: the Serial Number
5. Go Here: http://checkcoverage.apple.com
6. Enter: the Serial Number

It's information will then pop right up.

B. "is it hard to factory reset"

The only thing I would consider hard about it is the time it can take. You may need to contact your carrier. Here is the link: Restore your iPhone, iPad, or iPod to Factory Settings - Apple Support. If you have no computer, go here: How to Erase your iPhone, iPad, or iPod touch - Apple Support. I will take a bit long with out a computer.
