Why is there no camera in Notes?

Make sure the note you are in was started in the iCloud or on My iPhone Group / Folder.

Other email accounts that do support notes may not support the extra features and as such the camera may not show up in Notes started in those groups.

Tap the icon at the bottom right.

In the next screen tap on Camera.

Correct, Microsoft's Exchange does not support the extra Notes features.
