Use the support document you posted form to setup a service appointment at nearby Apple Store or Authorized service provider to have your iPhone looked at and repaired if required.

Please define "damaged".

If the hardware is physically damaged, it will have to be repaired or replaced.

Make an appointment at the Genius Bar of your local Apple Store or take it to your local authorized service provider.

Since you did not provide any information about it., get an appointment at an Apple store for an evaluation.
