How do I add a streaming service to my Apple TV menu

One of my streaming services, Crave TV, has disappeared from my Apple TV menu. How can I get it back?

As I said, if it's available you will find it.

A quick Google search found that the streaming service removed it from Apple TV nearly a year ago for the 3rd Generation Apple TV, and it may have been removed for your 2nd Generation model prior to that.
