What is your model Apple TV? Identify your Apple TV model - Apple Support

At what iOS software version was your iPhone when it last worked?

Have you tried setting the Maximize Compatibility option for the hotspot? If Personal Hotspot is not working on your iPhone or iPad (Wi-Fi + Cellular) - Apple Support

What message does the Apple TV give after trying to connect?
