With regards to the microphone icon that's appearing when accessing the address bar, this may be Dictation.

If you haven't yet, try heading to Settings > General > Keyboard > Turn off 'Enable Dictation'. Details on this here: Dictate text on iPad - Apple Support

If you haven't already done so, you can also attempt to clear the history and cookies in Safari using the steps from this article: Clear the history and cookies from Safari on your iPhone, iPad, or iPod touch
