Troubleshooting Disassociating Devices:

A. Verify its Removal:

See if it is shown in your list of devices. If not, then it has been successfully removed: checkcoverage.apple.com

B. New iCloud Sessions:

Log out of iCloud on all of your devices, and then log back in. Perhaps it all needs to resync.

C. About the Apple ID:

The Apple ID would be an eMail address. So, if you've only have a few eMail addresses, then it would be one of those.
