When I go to settings→privacy→App Privacy Report I see that camera has accessed my photos. Sometimes I see the green dot on my screen when I’m On the phone but it is for a short period of time. I have denied apps that have requested access to the camera
You probably used a 3rd party app which you gave access to camera and camera roll to.

Of course the camera accesses your photos… how else do you expect it to save the pictures you take?
