Log out on old device

I received an email this morning that my old Apple ID was used to log into an iPhone 5. How do I log out of that if I don’t even remember my old login information?

Try to recover your password using this link If you have forgotten your Apple ID password – Apple Support (UK)

and after that log out. You might want to review the following information as well
