What you're describing sounds like a badge icon over the Phone app on your iPhone. This is a type of notification. You can manage notification settings for the Phone app under Settings > Notifications > Phone. If you'd prefer, you can toggle off "Badges."

If you'd like to leave Badges on for the Phone app, but remove this specific badge notification, you can open the app, and tap the tab of the Phone app the notification is for (Recents or Voicemail).
