Is it possible to use different credit cards for different items? If my iCloud monthly fee is paid for, say, by my Visa card, can I charge a monthly app fee to my Apple Card? If so, where do I set that up?

Whatever card you have added to your apple account (your Apple ID appleid.Apple.com) is charged for your App Store and iTunes purchases. You can change the card but you cannot have multiple cards from which to choose.
