Calls not staying on hold

Hi, I usually always make phone calls and have to call others and place my initial call on hold. Or I may be on the phone and answer another call placing the person on hold. But recently it’s been hanging up without even putting them on hold or it only lasts about 5 seconds before it hangs up on the other person. Can someone tell me how to fix this?
