How to file a claim with AppleCare+ for my stolen iPhone?

my i phone 12 pro was stolen in lahore pakistan and it was secured with apple care plus. i have also registered an FIR in this regard but i am not able to find my phone. kindly help me in this regard to claim my phone through Apple care plus. i also had its box and its bill. since, apple is always there to give facilities to its customers, so following that legacy kindly help me to register my claim
