My iPhone 12 won't get new keychain items created on my macbook, my macbook gets passwords created on my iphone but not the other way around, turning off sync on both devices doesn't help.

When I create a keychain item on my macbook, my iPhone doesn't sync it, but a password created on my iPhone is synced to my macbooks keychain. Turning off/on syncing on both devices doesn't help. This has plagued me for years, how do I resolve this?

MacBook Pro 16″, macOS 14.4
